Admitting Information

Surgical Patients

Your doctor may set up a pre-admit appointment prior to your surgery date. The appointment will take approximately one (1) hour. Please arrive 30 minutes prior to your scheduled appointment and bring your pre-admission packet, insurance cards and a government-issued photo ID.

On the day of your surgery, please report to the admitting office at the hospital or as directed by your pre-admission nurse.

Diagnostic Testing/Outpatient Procedures

Appointments should be scheduled in advance through our centralized scheduling department by calling 239-624-4443.

If you have not pre-registered for this admission, please bring the following on the day of your appointment:

  • Insurance cards
  • Government-issued photo ID
  • Doctor’s orders (if provided to you)

Your insurance co-payments, deductibles and out-of-pocket amounts will be collected on the day of your appointment. If you do not have insurance, we will be happy to help you apply for financial assistance.

Direct Admissions

Your doctor may send you directly to the hospital from home or from their office to be admitted. As a courtesy, you may call the admitting department to find out the status of your admission prior to arriving at the hospital.

Items are given to you upon your admission:

  • Patient Rights
  • Notice of Privacy Practices
  • Statement on Patient’s Hospital Visitation Rights & Rules
  • Florida Advance Directive

Room Assignments

NCH Healthcare System has semi-private and private rooms. Most patients are placed in a specific area with other patients who have similar illnesses or conditions. Sometimes a physician will request that a patient be placed on a specific floor where special monitoring equipment is available.

If you request a private room, we will make every effort to accommodate you. However, it is not possible to guarantee a specific accommodation in advance, as room assignments are based upon availability on the day of admission.

Money/Valuables

Please leave your jewelry, items of sentimental value, credit cards and cash at home. The hospital is not responsible for any valuables left at your bedside.

International Patients

We have for your convenience, an international insurance coordinator on site who is available to assist and answer any questions you may have regarding your international/travel insurance.
