Insurance and Billing

We will verify your health insurance coverage(s) that may cover some or all of the services to be performed, NCH Bonita Ambulatory Surgery Center will initiate contact with them to determine your cost-sharing responsibilities. Prior to surgery, someone from the center will contact you with your estimated cost-sharing out of pocket.

Upon request, a written estimate will be provided to you. This amount is due prior to or on the day of your surgery. Please remember to bring a form of payment with you. We accept cash, checks, major credit cards and care credit.

After surgery, NCH Bonita Ambulatory Surgery Center will submit your bill to your insurance company. Any additional financial responsibilities, above what was given as the estimate, will be balance billed to the patient.

NCH Bonita Ambulatory Surgery Center’s standard collection policy is to produce and send two statements to patients for their cost sharing amount. If payment is not received within 90 days of date of service, the balance will be turned over to collections.

NOTE: You will receive a separate bill from your doctor, anesthesiologist and/or pathologist.

Services are provided in this health care facility by other health care providers who may separately bill the patient and who may or may not participate with the same health insurers or health maintenance organizations as NCH Bonita Ambulatory Surgery Center.
